1 Year Firefighter Fee $100
Includes initial consultation, registration in CURT, and any necessary follow up visits within the 12 months of prescription. Complimentary electronic letter signed by prescribing physician, which can be submitted for department HR purposes, is included.
6 Month Fee $90
Includes initial consultation, registration in CURT, and any necessary follow up visits within the 6 months of prescription.
Probation Letter Service $80
